This form will be given to the applicant in 2 sets. Both sets are filled up and signed by the applicant prior to his/her scheduled interview at the US Embassy.

For fiancee'/K-1 visa applicants, the embassy includes the DS-156-K forms, but in our case, we only used the DS-156 form.

The DS-157 form (Supplement to NonImmigrant Visa Application) usually comes with the DS-156 in 2 copies as well.

These forms are downloadable and printable online for free. If you think you need extra forms as back ups, you can print many copies as you want.
