The Affidavit of Support (From I-864):

This is the form that the NVC will give the petitioner (for I-130 petition) once the immigrant visa fee will be paid and sent back by the petitioner to NVC.  The petitioner will then submit it back properly filled up and notarized. The form can also be downloaded for free at the USCIS website.

Take NOTE: Always download it directly from USCIS site if you want to have more back up copies, as USCIS forms are always updated from time to time. 

The actual form below was the most recent form we used for Ging's Adjustment of Status. We ran out of copies for the actual form we submitted for the year 2005, but anyway, the form looks like exactly the same except for the numerical data.
