This is to be used and is included together with the petition if the petitioner uses a lawyer to do all the paperworks. In our case, we hired one.

You don't need to hire a lawyer, unless perhaps you or your petitioner has a complicated case situation. Craig hired one because he hated paperworks! LOL...but one more important thing is he had to joggle so much of his time at work, and barely had a time to do the paperworks alone.
